Who qualifies
You must meet both a financial test and a medical test:
- Financial: Household gross income must be at or below 3× the federal poverty guideline for your household size. Example from Miracle Flights: a family of 4 making $75,000 may qualify; the same family at $100,000 will not.
- Medical: The child must have a confirmed appointment at a board-certified specialist or accredited treatment center that's too far to reach by car, and must be medically cleared to fly on a commercial airline.

How to apply
- Go to miracleflights.org/request-a-flight and fill out the Child Medical Flight Request Application.
- Ask your child's treatment site to submit the Treatment Site Verification Form directly to Miracle Flights (the family cannot fill this out — it must come from clinic staff).
- Upload financial documents: the first two pages of your most recent IRS Form 1040, plus any SSI/SSDI award letters, child-support records, or self-employment schedules (Schedule 1, C, E, or F) that apply.
- Submit everything at least 14 days before your departure date. Late requests are not guaranteed.
- Questions or help with the form: call 1-800-359-1711 or email info@miracleflights.org.
Common pitfalls
- Don't wait until the last minute — the 14-day rule is firm. Apply as soon as the appointment is scheduled.
- The treatment site form is on the clinic, not you. Call the appointment scheduler and ask them to send it; many hospitals have done this before.
- They book commercial flights only — not air ambulances or private jets. If your child can't fly commercial, ask the clinic about Mercy Medical Angels or a Children's Hospital social worker.
- Service dogs flying to a training facility have a separate application on the same page.
